Investigation of the circumstances surrounding the death of Jeffrey Epstein at MCC New York.
Key findings:
- Systemic staffing failures at MCC
- Chronic understaffing in the Special Housing Unit
- Guards working overtime shifts exceeding 16 hours
- Non-functional or poorly positioned security cameras
- Failure to follow suicide prevention protocols
- Inadequate mental health screening
The OIG made multiple recommendations for reform of BOP facilities.
Note: The OIG found no evidence that criminality caused Epstein's death.

The Epstein Case in 2023
In 2023, litigation over banks' dealings with Epstein reached settlements: Deutsche Bank agreed to pay $75 million to survivors, and JPMorgan Chase agreed to pay $290 million to survivors and $75 million to the U.S. Virgin Islands. On December 18, 2023, Judge Loretta A. Preska ordered that sealed records from Giuffre v. Maxwell identifying previously anonymized individuals be unsealed, setting up the January 2024 releases.
