METROPOLITAN CORRECTIONAL CENTER — INCIDENT REPORT Facility: Metropolitan Correctional Center (MCC), New York Inmate: Jeffrey Edward Epstein Register Number: 76318-054 Date of Incident: July 23, 2019 INCIDENT SUMMARY: On July 23, 2019, correctional staff at the Metropolitan Correctional Center discovered Jeffrey Epstein in his cell in a semi-conscious state with marks on his neck. The incident was initially reported as an apparent suicide attempt. IMMEDIATE RESPONSE: - Medical staff responded and provided emergency assessment - Epstein was transported to the hospital for evaluation - Epstein was placed on suicide watch in the facility's mental health unit - 24-hour monitoring was initiated per BOP suicide prevention protocols SUICIDE WATCH: Epstein was maintained on suicide watch for approximately six days, during which: - He was under continuous observation by staff - All potentially dangerous items were removed from his cell - Mental health professionals conducted regular assessments - Epstein was housed in a specially designed observation cell REMOVAL FROM SUICIDE WATCH (July 29, 2019): After approximately six days, MCC psychologists recommended removing Epstein from suicide watch: - Mental health staff assessed that Epstein was no longer at imminent risk - Epstein was returned to the Special Housing Unit (SHU) - He was placed on "psychological observation" (a lower level of monitoring) - He was assigned a cellmate as a safety measure SUBSEQUENT EVENTS: On August 10, 2019 — approximately 12 days after being removed from suicide watch — Epstein was found dead in his cell. His cellmate had been transferred the day before, and the guards assigned to his unit had fallen asleep.
